A Change Order Request form is a subcontractor's written request to a prime contractor to change or modify some part of the original plans or specifications or other contract documents. This could be change to the work, the price and/or deadlines for completion. Once approved by the prime contractor, the prime contractor will in return submit a change order to the project owner or owner's representative for approval.
